ความเป็นมา

ภาวะปากแหว่งเพดานโหว่และความพิการแต่กำเนิดของศีรษะและใบหน้า เป็นความพิการแต่กำเนิดที่เกี่ยวข้องกับปัญหาหลายด้านและมีความซับซ้อน ซึ่งมีผลกระทบต่อร่างกายและจิตใจของผู้ป่วย บิดามารดาและผู้ปกครอง รวมถึงการสูญเสียทางเศรษฐกิจของชาติ โดยความพิการทางร่างกายที่เห็นเด่นชัดคือ รูปร่างและเค้าโครงของใบหน้า การพูดไม่ชัด ภาวะแทรกซ้อนหูชั้นกลางอักเสบ การได้ยิน ระบบการกลืน ปัญหาเกี่ยวกับฟัน การสบฟัน รวมถึงพัฒนาการและการเจริญเติบโตช้า จากการศึกษาที่ผ่านมาพบอัตราการเกิดอุบัติการณ์ในผู้ป่วยปากแหว่งเพดานโหว่ในภาคตะวันออกเฉียงเหนือ ประมาณ 2.5 รายต่อเด็กแรกเกิด 1,000 ราย ซึ่งเป็นอัตราที่อยู่ในกลุ่มอุบัติการณ์ที่สูงที่สุดในโลก และสามารถประมาณการณ์ได้ว่าจะมีเด็กแรกเกิดใหม่ที่มีภาวะปากแหว่ง เพดานโหว่ ในภาคตะวันออกเฉียงเหนือมีอัตราสูงถึง 800 รายต่อปีหรือทั่วประเทศประมาณปีละกว่า 2,000 ราย

การให้การรักษาผู้ป่วยปากแหว่ง เพดานโหว่ ในประเทศไทยส่วนใหญ่ มักเป็นการรักษาที่มุ่งเน้นที่การผ่าตัด หรือการให้การดูแลโดยบุคลากรทางการแพทย์แต่ละฝ่ายแยกกัน โดยขาดการประสานงานในการวางแผนการรักษาและการให้การรักษาร่วมกันแบบทีมสหวิทยาการ ซึ่งทำให้ผลการรักษาผู้ป่วยปากแหว่ง เพดานโหว่ เป็นการรักษาเฉพาะปัญหา และทำให้บางปัญหาของผู้ป่วยถูกละเลย เกิดการให้การรักษาแบบซ้ำซ้อน หรือภาวะแทรกซ้อนโดยไม่จำเป็น ซึ่งเป็นการสูญเสียทางเศรษฐกิจและทำให้การฟื้นฟูสภาพของผู้ป่วยเป็นไปได้ไม่สมบูรณ์

การรักษาและฟื้นฟูผู้ป่วยปากแหว่ง เพดานโหว่ จำเป็นต้องอาศัยการรักษาและติดตามผลเป็นระยะเวลายาวอย่างต่อเนื่องจนผู้ป่วยโตเป็นผู้ใหญ่โดยสมบูรณ์ การดูแลแบบทีมสหวิทยาการโดยบุคลากรหลายสาขาที่เชี่ยวชาญเฉพาะร่วมกัน จะทำให้ผู้ป่วยได้รับการรักษาที่ดี ถูกต้อง เหมาะสม ลดการเกิดภาวะแทรกซ้อน การสูญเสียทางเศรษฐกิจ ทำให้ผู้ป่วยได้รับการฟื้นฟูสภาพโดยสมบูรณ์และสามารถดำรงชีวิตอยู่ในสังคมได้อย่างเป็นปกติสุข

โรงพยาบาลศรีนครินทร์ คณะแพทยศาสตร์ มหาวิทยาลัยขอนแก่น ซึ่งเป็นโรงพยาบาลระดับมหาวิทยาลัยแห่งแรกของภาคตะวันออกเฉียงเหนือ ได้มีการให้บริการรักษาพยาบาลผู้ป่วยมาตั้งแต่ปี พ.ศ. 2518 โดยมีการผ่าตัดผู้ป่วยปากแหว่งเพดานโหว่รายแรกในปี พ.ศ. 2521 ต่อมาได้มีการตระหนักถึงความสำคัญของการดูแลผู้ป่วยปากแหว่งเพดานโหว่อย่างต่อเนื่อง มีการจัดทีมบริการผู้ป่วย จากผู้เชี่ยวชาญหลายสาขาในลักษณะของทีมสหวิทยาการ ประกอบด้วยศัลยแพทย์ตกแต่งจากภาควิชาศัลยศาสตร์ กุมารแพทย์ ประสาทศัลยแพทย์ แพทย์โสต ศอ นาสิก พยาบาล และทันตแพทย์จากแผนกทันตกรรมของโรงพยาบาลศรีนครินทร์

ในปี พ.ศ. 2533 มีการบริการผู้ป่วยปากแหว่ง เพดานโหว่ ร่วมกับภาควิชาทันตกรรมจัดฟันของคณะทันตแพทยศาสตร์ มหาวิทยาลัยขอนแก่น ซึ่งได้รับการสนับสนุนในการฝึกอบรมบุคลากรจากประเทศออสเตรเลีย โดย Professor Keith Godfrey อดีตหัวหน้าภาควิชาทันตกรรมจัดฟัน มหาวิทยาลัยซิดนีย์ ประเทศออสเตรเลีย มีการจัดอบรมการดูแลผู้ป่วยปากแหว่งเพดานโหว่ขึ้น เป็นครั้งแรกในปี พ.ศ. 2535 และครั้งที่ 2 ในปีพ.ศ. 2537 จากนั้นก็มีการเปิดให้บริการทันตกรรมดูแลผู้ป่วยปากแหว่ง เพดานโหว่ ขึ้นที่คณะทันตแพทยศาสตร์ด้วย

ในปี พ.ศ. 2542 ได้มีความพยายามในการพัฒนาระบบการดูแลผู้ป่วยปากแหว่ง เพดานโหว่ โดยมีการรวมตัวกันของบุคลากรขึ้นในลักษณะของกลุ่มวิจัยในนามกลุ่มวิจัย “การบริการผู้ป่วยปากแหว่ง เพดานโหว่ และความผิดปกติของใบหน้าแบบสหวิทยาการ” (Multidisciplinary management of cleft lip, cleft palate and craniofacial anomalies) โดยมีหัวหน้ากลุ่มคือ รศ.นพ.บวรศิลป์ เชาวน์ชื่น หัวหน้าหน่วยศัลยศาสตร์ตกแต่ง ภาควิชาศัลยศาสตร์ คณะแพทยศาสตร์ และมีทีมสหสวิทยาการที่เกี่ยวข้อง เช่น สูติแพทย์ กุมารแพทย์ ศัลยแพทย์ตกแต่ง ประสาทศัลยแพทย์ โสตศอนาสิกแพทย์ ทันตแพทย์จัดฟัน ทันตแพทย์ศัลยศาสตร์ช่องปากฯ วิสัญญีแพทย์ จิตแพทย์ นักแก้ไขการพูด นักแก้ไขการได้ยิน พยาบาล นักสังคมสงเคราะห์ และนักชีวสถิติ โดยมี Professor Keith Godfrey ผู้ซึ่งมีประสบการณ์ในการจัดการระบบการดูแลผู้ป่วยปากแหว่งเพดานโหว่ จาก Royal Alexandra Hospital for Children ประเทศออสเตรเลีย เป็นที่ปรึกษา โดยในระยะแรกได้รับการสนับสนุนงบประมาณในการดำเนินกิจกรรมจากฝ่ายวิจัย คณะแพทยศาสตร์ มหาวิทยาลัยขอนแก่น กลุ่มวิจัยดังกล่าวได้ดำเนินกิจกรรมที่เกี่ยวข้องกับการดูแลผู้ป่วยปากแหว่ง เพดานโหว่ทั้ งในด้านการวิจัยการบริการผู้ป่วย และการจัดกิจกรรมวิชาการอย่างต่อเนื่อง มีการตั้งคลินิกพิเศษให้บริการผู้ป่วยปากแหว่ง เพดานโหว่แบบสหวิทยาการในทุกวันศุกร์ที่ 4 ของเดือน

ในปี พ.ศ. 2543 ได้มีการจัดตั้ง “ศูนย์การดูแลผู้ป่วยปากแหว่ง เพดานโหว่และความพิการแต่กำเนิดของใบหน้าและกระโหลกศีรษะขึ้น โดยได้ทำความร่วมมือและได้รับการสนับสนุนจากองค์กร The Smile Train Charity Organization เพื่อช่วยพัฒนาระบบการดูแลผู้ป่วยและการผ่าตัดรักษาผู้ป่วยปากแหว่ง เพดานโหว่ ภายใต้ชื่อโครงการ “The Smile Train Cleft Care Project: Khon Kaen University Cleft Center”

กิจกรรมที่ดำเนินการโดยทีมงานสหวิทยาการดังกล่าว เป็นกิจกรรมที่มุ่งเน้นด้านการพัฒนาระบบการดูแลผู้ป่วยที่มีประสิทธิภาพ เพื่อพัฒนาคุณภาพชีวิตของผู้ป่วยปากแหว่งเพดานโหว่ และความพิการแต่กำเนิดของกะโหลกศีรษะและใบหน้า โดยมีกลยุทธ์ที่มุ่งเน้นด้านการมีส่วนร่วมของผู้ป่วย ครอบครัว และผู้เกี่ยวข้อง การพัฒนาคุณภาพของกระบวนการดูแลผู้ป่วยทั้งการป้องกันและรักษาที่มีประสิทธิภาพ มีความคุ้มค่า โดยดำเนินการภายใต้ชื่อ “โครงการตะวันฉาย” ซึ่งเป็นโครงการที่ได้รับพระราชทานพระราชานุญาตให้เป็นโครงการเฉลิมพระเกียรติ สมเด็จพระเทพรัตนราชสุดาฯ สยามบรมราชกุมารี ในวโรกาสที่ทรงเจริญพระชนมายุ 48 พรรษา ซึ่งประกอบด้วยการดำเนินงาน 9 กิจกรรม ดังนี้

การจัดตั้ง “สมาคมความพิการปากแหว่ง เพดานโหว่ ใบหน้าและศีรษะ แห่งประเทศไทย”
การจัดตั้ง“ศูนย์การดูแลผู้ป่วยปากแหว่งเพดานโหว่ และความพิการแต่กำเนิดของศีรษะและใบหน้า มหาวิทยาลัยขอนแก่น”
การจัดตั้ง “ชมรมพยาบาลเพื่อการดูแลสร้างเสริมสุขภาพครอบครัวเด็กปากแหว่งเพดานโหว่
การจัดตั้ง“ชมรมผู้ปกครองผู้ป่วยปากแหว่งเพดานโหว่ และความพิการแต่กำเนิดของศีรษะและใบหน้า
การก่อตั้ง “กองทุนตะวันฉาย” กองทุนเพื่อช่วยเหลือครอบครัวเด็กปากแหว่งเพดานโหว่ และความพิการแต่กำเนิดของศีรษะและใบหน้า
โครงการ“พัฒนาทักษะของผู้ปกครองเพื่อการดูแลป่วยปากแหว่งเพดานโหว่ และความพิการแต่กำเนิดของศีรษะและใบหน้า ในภาคตะวันออกเฉียงเหนือ”
โครงการประชุมเชิงปฏิบัติการ เรื่อง“การพัฒนาระบบและเครือข่ายของทีมงานการดูแลผู้ป่วยปากแหว่งเพดานโหว่ เพื่อพัฒนาคุณภาพชีวิตและสร้างเสริมสุขภาพเด็กไทยที่มีความพิการปากแหว่งเพดานโหว่ ที่ต้องการดูแลแบบองค์รวม และพัฒนาการด้านการพูดและภาษาในเขตชุมชนเมือง”
โครงการสัมมนาทางวิชาการนานาชาติ “การดูแลผู้ป่วยปากแหว่งเพดานโหว่และความพิการแต่กำเนิดของศีรษะและใบหน้าแบบสหวิทยาการ ประเทศไทย ครั้งที่ 1 พ.ศ. 2546”
โครงการวิจัยแบบสหสถาบัน“การศึกษาอุบัติการณ์และสาเหตุของภาวะปากแหว่งเพดานโหว่และความพิการแต่กำเนิดที่เกี่ยวข้องในประเทศไทย”

ในปีพ.ศ. 2546 ได้มีการจัดตั้งโครงการตะวันฉาย โดยภาควิชาศัลยศาสตร์ คณะแพทยศาสตร์ มหาวิทยาลัยขอนแก่น เพื่อให้บริการและพัฒนาระบบการดูแลผู้ป่วยปากแหว่ง เพดานโหว่ และความพิการแต่กำเนิดของศีรษะและใบหน้าในประเทศไทย เพื่อเป็นการเฉลิมพระเกียรติสมเด็จพระเทพรัตนราชสุดาฯ สยามบรมราชกุมารี และได้ขอพระราชทานพระราชานุญาตในการจัดตั้งโครงการ ซึ่งได้รับพระราชทานพระราชานุญาตในการจัดตั้งโครงการตะวันฉาย เมื่อวันที่ 24 พฤศจิกายน 2556 โดยมีวัตถุประสงค์ คือ

เป็นศูนย์กลางการประสานงานของบุคลากรของทีมสหวิทยาการในการดูแลผู้ป่วยปากแหว่ง เพดานโหว่ และความพิการแต่กำเนิดของศีรษะและใบหน้า มหาวิทยาลัยขอนแก่น
เป็นศูนย์กลางของการประสานงานเพื่อการพัฒนาระบบที่มีความเหมาะสมและมาตรฐานของการดูแลผู้ป่วยปากแหว่ง เพดานโหว่และความพิการแต่กำเนิดของศีรษะและใบหน้า โดยมุ่งเน้นผู้ป่วยและครอบครัวเป็นศูนย์กลาง การดูแลแบบองค์รวม และทีมสหวิทยาการ
เพื่อการนำเสนอการแลกเปลี่ยนความรู้ การจัดการศึกษา การฝึกอบรม การผลิตเอกสารทางวิชาการ และเป็นแหล่งความรู้แก่บุคลากรทางการแพทย์ นักศึกษา ผู้ป่วยและครอบครัว และผู้ที่สนใจ องค์กรอื่นๆ
การทำวิจัยที่เกี่ยวข้อง เพื่อนำผลงานที่ศึกษาวิจัย มาสร้างองค์ความรู้และชี้นำสังคม และลดปัจจัยการเกิดภาวะความพิการแต่กำเนิด
ส่งเสริมการทำงานแบบทีมสหวิทยาการและความร่วมมือแบบสหสถาบัน ทั้งในระดับประเทศและระดับนานาชาติ

หนังสือพระราชทานพระราชานุญาตในการจัดตั้งโครงการ “ตะวันฉาย”

ต่อมาเมื่อวันที่ 20 ธันวาคม พ.ศ. 2547 “ศูนย์การดูแลผู้ป่วยปากแหว่ง เพดานโหว่และความพิการแต่กำเนิดของศีรษะและใบหน้า มหาวิทยาลัยขอนแก่น ภายใต้โครงการตะวันฉาย” ได้รับพระมหากรุณาธิคุณโปรดเกล้าฯ จากสมเด็จพระเทพรัตนราชสุดาฯ สยามบรมราชกุมารี เสด็จเป็นองค์ประธานเปิด ศูนย์การดูแลผู้ป่วยปากแหว่ง เพดานโหว่ฯ ซึ่งปัจจุบัน เป็นที่รู้จักกันในวงกว้างในชื่อ “ศูนย์ตะวันฉาย”

จากนโยบายของคณะแพทยศาสตร์ ในการตอบสนองสุขภาวะของประชาชนในภาคตะวันออกเฉียงเหนือและกลุ่มประเทศภูมิภาคใกล้เคียง พร้อมทั้งตอบสนองต่อการเป็นโรงพยาบาลที่ผ่านการรับรองคุณภาพ (Hospital Accreditation) จึงได้มีการพัฒนาการดูแลสู่ความเป็นเลิศโดยการจัดตั้งศูนย์การดูแลเฉพาะทาง (excellent center) เพื่อให้การดูแลกลุ่มผู้ป่วยเฉพาะโรคเฉพาะทางโดยทีมสหสาขาวิชาชีพที่มีคุณภาพเพิ่มมากขึ้น ดังนั้น คณะ จึงมีนโยบายสนับสนุนในการจัดตั้งศูนย์บริการทางการแพทย์ชั้นเลิศ และตามมติที่ประชุมฯ เมื่อวันที่ 2 เมษายน พ.ศ. 2558 ณ ห้องประชุมสีฐาน สำนักงานผู้อำนวยการ จากบันทึกข้อความงานเลขานุการโรงพยาบาลศรีนครินทร์ ลงวันที่ 9 เมษายน พ.ศ.2558 ได้ให้ ศูนย์ตะวันฉาย ซึ่งเป็น 1 ใน 12 ศูนย์บริการทางการแพทย์ชั้นเลิศ นำเสนอข้อมูลแก่ผู้บริหารคณะฯ ในหัวข้อ 1) สิ่งที่ต้องการรับการสนับสนุน 2) แผนการดำเนินงาน 4 ปี (ปี 2558-2562) โดยนำเสนอในวันที่ 8 มิถุนายน พ.ศ. 2558 ณ ห้อง ห้องประชุมหนองแวง สำนักงานคณบดี ชั้น 6 และจากความเห็นชอบของสองคณะจึงได้ตั้งชื่อศูนย์ความเป็นเลิศ คือ “ศูนย์ความเป็นเลิศตะวันฉายเพื่อผู้ป่วยปากแหว่งเพดานโหว่ และความพิการแต่กำเนิดของศีรษะและใบหน้า มหาวิทยาลัยขอนแก่น (Tawanchai Center of Excellence for Patients with Cleft Lip Cleft Palate and Craniofacial Deformities, Khon Kaen University)” และเมื่อวันพุธที่ 1 กุมภาพันธ์ 2560 ศูนย์ตะวันฉาย ได้ทำพิธีเปิดป้าย ศูนย์ความเป็นเลิศตะวันฉายฯ ซึ่งได้รับเกียรติจาก ดร.ณรงค์ชัย อัครเศรณี นายกสภามหาวิทยาลัยขอนแก่น พร้อมด้วยอุปนายกสภามหาวิทยาลัยขอนแก่น ผู้บริหารมหาวิทยาลัยขอนแก่น ผู้บริหารโรงพยาบาลศรีนครินทร์ ผู้อำนวยการสำนักงานสาธารณสุขจังหวัดขอนแก่น คณะกรรมการศูนย์การดูแลผู้ป่วยปากแหว่ง เพดานโหว่ฯ คณะกรรมการศูนย์วิจัยผู้ป่วยปากแหว่ง เพดานโหว่ฯ คณะกรรมการมูลนิธิตะวันฉาย แพทย์และพยาบาลทีมสหวิทยาการศูนย์ตะวันฉาย รวมถึงผู้ป่วยและญาติผู้ป่วยที่มีภาวะปากแหว่งเพดานโหว่ฯ เข้าร่วมเป็นสักขีพยานในการเปิดป้ายศูนย์ความเป็นเลิศตะวันฉายฯ และเยี่ยมชมนิทรรศการผลงาน ณ ศูนย์ตะวันฉาย อาคารคลินิกทันตกรรม 1 คณะทันตแพทยศาสตร์ มหาวิทยาลัยขอนแก่น

Background

Cleft Lip and Palate and Craniofacial Deformities are among the most common birth defects that could lead to many problems, and has many complications, which can effect on both the physical and mental health of the patients, parents and their families. It can also lead to economic loss of the country. The obvious physical deformities that we could see are facial deformation, slurred speech, otitis media complications, hearing problems and ear infections, swallowing problems, nasal speaking voice, dental and occlusal problems, and growth delays. According to previous studies, the incidence rate of Cleft Lip and Palate in the Northeastern region of Thailand was about 1.93 per 1000 newborns, which is the highest incidence rate in the world, and it could be estimated that there will be as high as 800 newborns with cleft lip and palate in the Northeast and 2000 cases all over the country per year.

In the past, treatments of Cleft Lip and Palate patients in Thailand were mostly done by a surgery, and/or the patient received care by an independent team of medical personnel, meaning each department work individually and had lack of coordination in treatment planning and interdisciplinary team treatment making cleft lip and palate treatment in the past inefficient and left some of the problems untreated, this lead to treatment redundancy, sometimes this could even lead to patients having unnecessary complications and patient’s growth delays and also substantial economic loss.

Treatment and rehabilitation of patients with Cleft Lip and Palate requires a continuous and long-term follow-up treatments until the patients are fully grown. This must be done by a multidisciplinary team. Specialized personnel in many fields would work together and have instruments to measure the holistic treatment outcomes from different periods of time until the end of the treatment. This will allow the patient to receive a correct and appropriate treatment to reduce the incidence of complications and allows the patient to be completely rehabilitated and able to live normally in the society.

Srinagarind Hospital under the Faculty of Medicine, Khon Kaen University which is the first University Hospital of the Northeast region. The hospital has been in operation since 1975, later on, there was an awareness of the importance of continuous care for cleft lip and palate patients. This lead to the creation of the multidisciplinary team specialized on treating patient with cleft lip and palate. The team consists of plastic surgeons from the Department of Surgery, pediatricians, neurosurgeons, otolaryngologists, nurses and dentists from the dental department of Srinagarind Hospital.

In 1990 a treatment and services for patients with cleft lip and palate was provided, in coordination with the Department of Orthodontics of the Faculty of Dentistry, Khon Kaen University, which has been sponsored by Professer Keith Godfrey, former Division of Orthodontics Head of The University of Sydney. The first Thai Cleft Meeting was held in 1992 and the second one in 1994 then there was a dental service for cleft lip and palate patients at the Faculty of Dentistry, shortly after that, a dental service for cleft lip and palate patients at the Faculty of Dentistry was created.

In 1999, there was an attempt to develop a multidisciplinary management system to take care of patients with cleft lip palate and craniofacial deformities. Dr. Bawornsilp Chowchuen (position at that time) Head of Plastic Surgery Department, the Faculty of Medicine along with other founding members from other branches, such as Obstetricians, Pediatricians, Plastic surgeons, Neurosurgeons, Otolaryngologists, Orthodontist Dentists, Oral and Maxillofacial surgeons, Anesthetists, Psychiatrists, Speech Therapists, Nurses, Social workers and Biostatisticians, with Professor Keith Godfrey, who has experience managing the care system for patients with Cleft Lip and Palate from the Royal Alexandra Children’s Hospital, Australia as a consultant. In the first phases, the Center received budget for carrying out activities from the Research Department of the Faculty of Medicine. Khon Kaen University. The group has conducted activities related to treatment of patients with Cleft Lip and Palate whether it was researches, patient services, and the group has continuously organized academic conferences. A special Clinic was arranged every 4th Friday of the month to give advices to patients with Cleft Lip and Palate.

In 2000, a Center for Patients with Cleft Lip Cleft Palate and Craniofacial Deformities was created, with co-ordination and support from The Smile Train Charity Organization to help develop a system to take care of patients with Cleft Lip and Palate under the name “The Smile Train Cleft Care Project: Khon Kaen University Cleft Center”

The activities carried out by aforementioned multidisciplinary team are focused on developing the system to take care of patients with cleft lip and palate efficiently to improve the quality of living of the patients. The system has a strategy that focused on the involvement of patients, families, and staff. “Tawanchai Project” is the improvement of the quality and efficiency of patient care processes in both treatment and prevention. This project is founded under the royal permission on the Occasion of Her Royal Highness Princess Sirindhorn’s 48th Birthday Anniversary, which consists of 9 activities as follows:

The founding of The Thai Cleft Lip-Palate and Craniofacial Association.
The founding of Tawanchai Center for Patients with Cleft Lip Palate and Craniofacial Deformities, Khon Kaen University.
The establishment of Nurses’ Club for Promoting Family Health for Children with Cleft Lip and Palate.
The establishment of the “Parents of Patients with with Cleft Lip Palate and Craniofacial Deformities Club”
The founding of “Tawanchai Funds”, a fund aimed to financially help Patients with with Cleft Lip Palate and Craniofacial Deformities.
“Developing Parental Skills for the Care of Patients with Cleft Lip, Cleft Palate, and Craniofacial Deformities in the northeast” Project.
Arranging “Development of systems and networks of care teams for cleft lip and palate patients To improve the quality of life and promote the health of Thai children with cleft lip and palate disabilities who want holistic care and development of speech skills and language in urban areas” Workshops.
International seminar on “Taking multidisciplinary care of patients with cleft lip and palate and craniofacial deformities, Thailand, 2003”

2003 Marks the foundation Tawanchai Project, the project was launched by Department of Plastic Surgery, The Faculty of Medicine, Khonkaen University to offer services and care to patients with Cleft Lip and Palate in Thailand. The Project is launched to Celebrate in Honour of Her Royal Highness Prince Sirindhorn and the department had asked for royal permission to officially establish the project which the department received on November 24, 2013 with the following objectives:

To be a coordination center of personnel of the multidisciplinary team in the care of patients with cleft lip, cleft palate, and craniofacial deformities, Khonkaen University.
To be the center of coordination for the development of appropriate systems and standards of care for patients with Cleft Lip, Palate and Craniofacial Deformities by focusing on the patients and family using The Center’s holistic care system and multidisciplinary team.
To publish, present and exchange related studies, conduct educational management, training, and publication of academic documents and is a source of knowledge for medical personnel, students, patients and their families. and those interested, and other organizations.
To conduct relevant researches and using the study results to create a body of knowledge and guide the society and reduce the incidence of anomalies.
To promote multidisciplinary teamwork and multi-institute cooperation both nationally and internationally.

Subsequently, on December 20, 2004, the Center received grace from Her Royal Highness Princess Maha Chakri Sirindhorn and she presided over the opening of Cleft lip and palate care center, which is now widely known as “Tawanchai Center”

From the policy of the Faculty of Medicine in response to the health of the people in the Northeast and neighboring regions as well as responding to being a quality accredited hospital, the center has been developed into a specialized center also known as an “Excellent Center) to take care of patients with deceases that needs medical specialists from a multidisciplinary team to give appropriate treatments, therefore, improving the care center is aligned with The Faculty’s policy after the accreditation. As a result, the resolution of the meeting on 2 April 2015 in Si Than Conference room, Khonkaen University Office of the President and from the memorandum of secretary in Srinagarind Hospital dated April 9, 2015, that Tawanchai Center, which is 1 of 12 excellent medical centers, Presenting information to the faculty administrators under the topic 1) what needs to be supported 2) the 4-year action plan (2015-2019) which presented on June 8, 2015 at the Nong Waeng Meeting Room, The Dean’s Office, 6th floor, and with the approval of the two faculties, the Center of Excellence was named “Tawanchai Center of Excellence for Patients with Cleft Lip Cleft Palate and Craniofacial Deformities, Khonkaen University” and on Wednesday, February 1^st 2017, the Tawanchai Center performed the opening ceremony of “Tawan Chai Center of Excellence” which was honored by Dr. Narongchai Akkraseranee, President of Khon Kaen University Council along with Vice President of Khon Kaen University Council, Khon Kaen University administrators Executives of Srinakarin Hospital, Director of the Khon Kaen Provincial Public Health Office, Cleft lip and Palate Care Center committees, Cleft Lip and Palate Research Center committees, Tawanchai Foundation committees, multidisciplinary team of doctors and nurses at Tawanchai Center including patients and relatives of patients with Cleft Lip and Palate joined to witness the opening of the Tawanchai Center of Excellence and visit the exhibition of works at the Tawanchai Center Dental Clinic Building 1, The Faculty of Dentistry, Khon Kaen University
